Sylvia Plester-Silk is committed to protecting client privacy.

Collection of Personal Health Information

Sylvia collects personal health information directly from the client or from the person acting on their behalf. This includes name, date of birth, and a brief biopsychosocial history. Sylvia Plester-Silk also keeps a record of the care received during sessions. Occasionally, personal health information may be collected from other sources, with the client's consent and within the boundaries of the law.

Clients may request to see the contents of their file for a fee, and may request that information be corrected or an addendum of client’s opinions, where they differ from what is stated in the file, be added to the file where it is not possible to amend the file itself. In accordance with the Law, the client must provide a written letter requesting to view or make changes to their file.

This information will be retained for a maximum period of seven years.

Use and Disclosure of Personal Health Information

Sylvia Plester-Silk collects and discloses only the health information required to:

To provide offer holistic energy therapy services
Obtain payment for care (i.e., private insurance coverage forms)
Plan, administer and manage internal operations
Improve services
Comply with legal requirements
Fulfill a client’s request pertaining to their personal health information
And for any other purpose as required by the Law.

Security

Sylvia Plester-Silk uses reasonable precautions to keep her client’s personal health information secure.

She will not trade or sell the client's personal information. Sylvia obtains express consent to share personal health information with third parties. The following exceptions apply:

All Canadian citizens (including counselors) are required by law to report any suspicion of risk of child abuse or neglect. If Sylvia Plester-Silk believes that a child is at risk of being abused, has already been abused, or that someone who previously abused children is still a threat (for example, still working in a child-care setting) Sylvia must report the information to appropriate child protection authorities.
If Sylvia Plester-Silk has reason to believe that a person’s life may be in danger, in cases of suicidal or homicidal risk, the situation will be reported to the appropriate authorities.
Courts of law have the power to subpoena or demand that a therapist release client records if clients are involved in a court case in which their relationship with Sylvia Plester-Silk is deemed somehow relevant.

On occasion, Sylvia Plester-Silk may send out unsolicited notices to clients’ email accounts to provide counseling follow-ups, request evaluation of services, or to announce upcoming workshops and/or new services. Sylvia Plester-Silk will not send unsolicited messages regarding any third party commercial offers or advertisements.
